Nursing regulation across North America has moved firmly toward competency-based testing, and Canadian nursing graduates writing the NCLEX-RN in 2026 are stepping into an exam that looks different from the one their instructors may have written a decade ago.<\/p>\n

Starting in 2023, as part of the NCSBN’s Next Generation NCLEX (NGN) implementation, there will be multiple new ways to assess clinical judgment, including case studies and new test item formats. As a result of the NCLEX Changes 2026<\/a>, candidates can no longer rely on older practice materials, as they will need to be proficient in both traditional and Next Generation test formats.<\/p>\n

As a result of the changes, many Canadian candidates attempting to navigate multiple jurisdiction-specific licensing pathways have been asking: what types of questions will candidates see on the NCLEX-RN, and how do they compare to the previous format?<\/p>\n

In short, the current NCLEX combines traditional item formats with Next Generation NCLEX (NGN) questions. The NGN is the updated version of the NCLEX that incorporates clinical judgment and advanced patient-care scenarios. The most common reason applicants do not perform to the best of their clinical ability on the exam is inadequate preparation, including a lack of familiarity with either question format.<\/p>\n

This blog aims to explain the different types of NCLEX questions, the traditional vs. Next Generation NCLEX question formats, and most importantly, how to successfully answer them, in the context of a Canadian nursing student.<\/p>\n

Why Understanding NCLEX Question Types Matters<\/h2>\n

Every NCLEX question type assesses particular skills. Standard multiple-choice questions assess recall and basic application. Extended multiple-response questions assess the ability to distinguish between closely related clinical choices.<\/p>\n

NGN case studies assess the ability to complete a full clinical reasoning cycle, including the recognition and analysis of cues, the prioritization of hypotheses, the generation of solutions, and the implementation and evaluation of actions. Considering the critical importance of clinical judgement in nursing<\/a>, all NGN questions assess the ability to make real-world decisions and to evaluate clinical judgement.<\/p>\n

For Canadian nursing students, this is important for another reason: understanding the differences between the NCLEX-PN and NCLEX-RN<\/a> allows students to tailor their preparation, as the NCLEX-RN is only one of several pathways to nursing registration.<\/p>\n

Depending on the province and the role, students may also have to write the REx-PN or the CPNRE<\/a>, each with its own question framework. NCLEX-RN-specific question types help students understand that not all nursing registration examinations share the same structure, allowing for more efficient preparation.<\/p>\n

Understanding question types also lowers cognitive load in the testing environment. If students have never seen a bowtie question or a matrix\/grid question, they will spend a lot of time on test day trying to figure out the format, which is a missed opportunity that can be avoided with adequate preparation.<\/p>\n
